 Анотація: у даній статті розглянуто та проаналізовано один із актуальних методів викладання англійської мови, а саме case-study метод, як один з ефективних у засвоєнні іноземної мови, що допомагає у вивченні нової лексики та сприяє формуванню всебічно розвиненої особистості з критичним мисленням.

 Ключові слова: case-study метод, методика викладання, англійська мова.

 

 З підвищенням рівня освіти в Україні все більше іноземних студентів зацікавлені в навчанні в українських вузах, зокрема в медичних інститутах та на медичних факультетах провідних вузів нашої держави. Це зумовлює застосування нових та ефективних методів викладання, зокрема англійської мови, як найбільш поширеної мови викладання у вузах для іноземних студентів різних національностей.

 На сьогоднішній день, перед викладачем іноземної мови постає важлива проблема підготовки освіченої та високоморальної особистості з розвинутим критичним мисленням, особливо якщо ця людина медик.

 Вирішення цього завдання передбачає знаходження найефективніших та найактуальніших методів навчально-виховного процесу. Саме це й породжує актуальність нашого дослідження, а точніше, використання інтерактивного методу навчання – кейс-методу.

 Вперше метод кейс-стаді був застосований в Гарвардській школі бізнесу в 1924 році і він швидко набув популярності. Розроблений англійськими науковцями М. Шевером, Ф. Едейем та К. Єйтс. Методом аналізу ситуації, як він іншими словами називається, використовується як один з головних у різних сферах наук: економіці, юриспруденції, соціальних науках, філології та медицині. Конкретно у нашому випадку, він є одним з головних на уроках англійської мови як іноземної для студентів-медиків вищого навчального закладу.

 Метод case study - це активний метод навчання, який вимагає безпосередньої участі студентів під час уроку. Метод вивчення конкретних питань дає студентам прекрасну можливість творчо застосувати мовний матеріал на основі їх професійних знань та дозволяє їм адаптуватися до реальних ситуацій.

 Метод кейс об'єднує два елементи: сам випадок і обговорення цього випадку. Кейси забезпечують багатий контекстний спосіб введення нового матеріалу та створення можливостей для студентів застосувати на практиці те, про що вони тільки що дізналися на уроці. Ефективні кейси, як правило, засновані на реальних подіях та випадках. Людина, що приймає рішення, з якими стикається у певній ситуації, може вибирати між кількома альтернативами і кожна з цих альтернатив може бути підтримана логічним аргументом. Під час дискусії учні проводять аналітичну роботу з пояснення зв'язків між подіями у цій справі, визначають варіанти, оцінюють вибір та прогнозують наслідки дій.

 Випадки можуть бути різними за обсягом та змістом. Це залежить від цілей навчання іноземної мови, рівня студента, а також типу необхідної допомоги у процесі прийняття рішення. У нашому випадку ціль буде практична, яка відображає реальні життєві ситуації, так як для нас актуальним є навчання іноземної мови для студентів-медиків.

 Навчальне завдання практичного кейсу зводиться до підготовки лексики та граматики, вивченої раніше, практикування та прийняття рішень в конкретній мовній ситуації. Навчальний процес повинен відображати типові мовні ситуації, які частіше з'являються в реальному житті майбутніх медиків, тобто їх професійної підготовки. 

 Даний тип уроку допомагає розвивати та підвищувати критичне мислення студента; розвивати навички вирішення проблем; покращити організаційні навички студента; підвищити комунікативні навички; заохочувати спільне навчання та навички роботи з командою; пов'язати теорію і практику; змусити студентів бути активними під час уроку.

 Метод кейс уроку зазвичай передбачає три етапи: 

  • індивідуальна підготовка; 
  • невелика групова дискусія; 
  • велика група чи класна дискусія.

 За допомогою підручника Л.Я. Аврахової “English for medical students” ми підготували план уроку, кейс-завдання в якому чітко розкривають хід уроку та формують практично-критичні навички студента, розкривають нові знання мови та вказують на специфіку медичної дисципліни.

Тема: History of a patient

Завдання кейс-уроку полягають у: 

  • набутті й розвитку навичок застосування теоретичного матеріалу для аналізу практичних проблем; 
  • формуванні навичок пошуку, відбору й оці¬нюванні інформації; 
  • навчанні формулювати запитання й форму¬вати запити; 
  • формуванні навичок всебічного аналізу си¬туацій, прогнозуванні способів розвитку ситуацій; 
  • набутті умінь і навичок конструктивної критики

 І. Підготовка до сприйняття нового матеріалу:

  3. Мовна зарядка - обговорення теми в загальному.

 І. 1. T: Good morning students! How are you?

 2. T: Today we are going to speak about the history of patient illness, how to take the history. We will get acquainted with the new notions of HPC (History of presenting complaint), HPI (History of present illness), SE (Systemic enquiry), DH (Drug history), ALL (allergies) etc.

 3. T: Before we start speaking about patient history in general, I would like to get known about the specifics of history taking in your countries – India, Nigeria, Ghana, Palestine.

 GENITOURINARY - relating to, affecting, or being the organs of reproduction and urination

 GASTROINTESTINAL - of, relating to, affecting, or including both stomach and intestine

 VERTIGO - disordered vertiginous movement as a symptom of disease in lower animals

 ABDOMEN - the part of the body between the thorax and the pelvis

 THORAX - the part of the mammalian body between the neck and the abdomen; also its cavity in which the heart and lungs lie

 PELVIS - a basin-shaped structure in the skeleton of many vertebrates that is formed by the pelvic girdle and adjoining bones of the spine

 ULCER - a break in skin or mucous membrane with loss of surface tissue, disintegration and necrosis of epithelial tissue

 ADENOMA - a benign tumor of a glandular structure or of glandular origin

 HEARTBURN - a burning discomfort behind the lower part of the sternum due especially to spasmodic reflux of acid from the stomach into the esophagus

 FITS - a sudden violent attack of a disease (as epilepsy) especially when marked by convulsions or unconsciousness [3].

Taking a History

 Taking a patient's history is the initial part of clinical examination and its main aim is to find out the patient's present problem and how it affects the quality of their life. 

 The history is a review of the patient's current state of health and past medical condition. When taken carefully, it may give valuable information about the nature of the patient's problem and provide the necessary clues to help the doctor establish a preliminary or differential diagnosis. 

 The history-taking process is a well-established and commonly used sequence. 

 1 History of presenting complaint (HPC). The main symptoms should be clearly defined as soon as possible, to find out the cause of admission or seeking medical advice. The onset, severity, progression, associated features or symptoms are all important. A special focus is also made on pains associated or not with specific organs. 

 2 History of present illness (HPI). The patient is requested to give an account of recent events in their own words which in this way may be recorded in the history sheet. 

 3 Systemic enquiry (SE) also known as the review of systems (RoS). The history is taken of the main symptoms of the major bodily systems: General: mood, fatigue, anorexia, fever, night sweats, rashes, heat/ cold intolerance. Cardiovascular system (CVS): chest pain, palpitations. Respiratory system (RS): shortness of breath, cough, sputum, wheeze, hemoptysis. Gastrointestinal system (GS): nausea, vomiting, indigestion, abdominal pain, heartburn, change in bowel habit. Genitourinary system (GUS): nocturia, frequency, incontinence, change in color/smell of urine, menstrual difficulties. Central nervous system (CNS): headaches, weakness, dizziness, fits, faints, vertigo. 

 4 Past medical history (PMH). Patients are asked about their previous medical/surgical diseases. 

 5 Drug history (DH) and allergies (ALL). Information is obtained on any medication prescribed, self-administered drugs. 

 6 Family history (FH) provides information about any predisposition to disease, and relevant information on relatives. 

 7 Social history (SH). Information is collected about the patient's occupational, social, personal factors, such as habits, employment, housing, interests, sports, hobbies, physical exercise, the use of alcohol, tobacco, recreational drugs. 

 To accomplish the purpose, a set of the following practical tips has been developed. 

  • Show the patient your attention. 
  • Start by eliciting the presenting complaint. 
  • Let the patient tell story in their own words. 
  • Try not to interrupt. 
  • Use the language which the patient understands. 
  • Summarize the story for the patient to check, correct and add more relevant details. 
  • Obtain the patient's history also from other sources of information. 

 Having completed history taking, the doctor will perform the next stage of clinical examination, which is physical examination. The diagnostic process will proceed, but the first clues have already been obtained to formulate a preliminary diagnosis, which will help the doctor to develop their own approach to the patient's problem during physical examination [1].

 Висновки: Підводячи підсумок вищезазначеної інформації, ми можемо стверджувати, що сила кейс-методу полягає в активній участі студентів. У випадку навчання, студенти стикаються з проблемою, перш ніж створювати структуру для її вирішення.
